Golden Little Star is proud to provide food and housing support services in New York through our work with SOMOS Community Care and Hudson Valley Care Coalition.
We help eligible Medicaid members connect to the services and support they need to live healthier, safer, and more stable lives. Our team provides care coordination, housing support, food box assistance, and food case management services for individuals and families who need help navigating resources in their community.
About Our New York Services
Golden Little Star provides New York-based social care services focused on food, housing, and case management support. We work with members, families, providers, and community partners to identify needs, coordinate services, and help members access available resources.
Our goal is to make the process easier for members by providing support, guidance, and follow-up throughout the service process.
Services We Provide
Housing Services
Golden Little Star helps eligible members with housing-related needs by providing support and case management. Our housing services may include: Housing navigation and support Help identifying housing needs Support with locating housing resources Assistance with housing-related referrals Connection to rental, utility, and move-in support resources when available Follow-up and care coordination to help members stay connected to services We understand that stable housing plays an important role in overall health and well-being. Our team works with members to help reduce barriers and connect them to the right support.
Food Box Services
Golden Little Star helps eligible members access food box support through New York Social Care Network services. Food boxes may help members and families who are experiencing food insecurity or need additional nutrition support. Our team can help with: Food box coordination Referral support Member communication and follow-up Helping members understand the process Making sure members stay connected to available food resources
Food Case Management
Our food case management services help members identify food-related needs and connect with available nutrition support. We work with members to understand their situation, complete required steps, and help coordinate services through the appropriate network. Food case management may include: Screening support Referral coordination Follow-up with members Connection to food and nutrition resources Support with service navigation Documentation and communication with network partners
Who We Serve
Golden Little Star serves eligible New York Medicaid members who may need help with food, housing, or social care support. Services are based on eligibility, network requirements, and approval through the appropriate Social Care Network.
